We accept and will file most major medical insurance plans as well as routine eye care plans.

Routine eye care plans include but are not limited to Eye Med, Superior Vision and Vision Service Plan (or VSP).  A routine eye exam is just that – it is a yearly comprehensive wellness exam to evaluate your eye health and prescribe any eyewear that may be necessary. If you are interested in or currently wear contact lenses, additional evaluation is required and may or may not be covered by your routine vision plan. Our knowledgeable staff will assist you with your routine benefit information once we acquire your authorization prior to your visit.

We also accept and will file most major medical insurance plans if your visit is symptom/problem focused or due to a medical condition. We utilize state of the art technology and equipment to assess, treat and provide you with optimum care for ocular health related issues.

Please note: Insurance is not guarantee of payment. Please be advised that if you are using insurance coverage for your visit, this is a contract between you and your insurance company… not our office. If your insurance company has not reimbursed our office in full within 60 days of the claim, you will be billed for the balance. If the insurance company then sends a check after you have paid in full, we will send the check directly to you.

Payment Options:

Payment is expected at the time of service including all co-pays, deductibles, and any other non-covered items.

We accept cash, personal checks, and most major credit/debit cards including: American Express, Discover Card, Master Card and Visa.

Do you participate in a flexible spending account at work?  If so, we gladly accept most of those cards as well.

Effective immediately, any appointments that are cancelled with less than 24 hours notice will be charged $25.00
